After detailing your research basis and reviewing the Theory of Logic Change Model linked in Resources, use the Kellogg Foundation Logic Model Guide to guide the development of your theory of logic change model and create a basic logic model diagram for your planned work and intended results.
Include your diagram in the body of this paper. Also, create an incidence reduction formula based on readings from the Conyne text (linked in Resources).
