Aviation fatalities are significantly lower in the last 30 years; however, investigation models have not kept pace. They remain essentially unchanged, linear, and fail to better explain the complex relations between humans and aviation systems.

New approaches are needed to better understand, analyze, design, and evaluate the increasingly complex aviation human-system integration environment. A comprehensive and systematic analysis of various approaches to error causation, methodologies of investigation, and beneficial interventions is needed to understand the complex problems encountered in today’s aviation landscape.

Aviation accident investigation including commercial and general aviation (military too for that matter) seeks to provide a better understanding of how to improve safety. The focus needs to be shifted to an understanding of the system in which accidents take place, rather than an undue emphasis on the end result.

At all times, there needs to be a fundamental understanding of exactly where and why an incident/s becomes an accident.

Given that there exists competition between the 2 components of performance safety and productivity;
