▪ Synthesize information about your professional College’s recording
requirements.
Steps to take:
1) Go to the OCSWSSW website (www.ocswssw.org) and open a copy of the
Code of Ethics and Standards of Practice, Edition 2 (by clicking on the logo on
right, midway down the home page)
2) Read completely the section entitled “Principle IV: The Social Work and Social
Service Work Record”
3) Watch “Case Management Video Practice” by clicking on this

Watch and / or listen to the 10-minute
interview and take notes for use to complete the assignment.
4) Draft your recording (1-1.5 page maximum, double spaced) for submission to
your professor
Instructions about writing your submission:
After watching the video, pretend that you were Jonathan (the interviewer) of the client Elaine and were required to record a progress note in the official client file immediately following the interview.
Here are some questions to ponder before completing the
assignment.
a) What information in the interview is important to record and what is not so
important?
b) If you, the SSW, were unavailable at work one day for some reason, what
information would your Supervisor need to read about in the file to be up to date
about this client and this interview?
c) If your agency was under an Accreditation review and accreditors looked at
your file entry for this interview, what information must be present?
d) If your client Elaine chose to read your note (and did so) about the session,
would it likely seem respectful and accurate to her?
