Summative Assessment task details and instructions
Personalised care as identified in the NHS Long Term Plan (2019) promotes the nurse to consider the ‘person’ as opposed to the ‘patient’ (NHS England, 2019; Nursing and Midwifery Council [NMC], 2018). In doing so, you will need to use appropriate communication skills and interpersonal skills to support and enable people to make informed choices within their episodes of care. This assessment will centre around a person’s story of an episode of care. You will need to choose one story/experience and address the assessment criteria set out under ‘Assessment Criteria’.

To FULLY meet the assessment criteria, you need to:
Briefly summarise the persons episode of care and describe the importance of partnership working between the nurse and person in relation to personalised care and support planning.

Explain how a nurse can use communication and interpersonal skills within this person’s episode of care, in identifying what matters to the person.

It is essential that you maintain any individual’s anonymity. You should use pseudonyms for all names: patient, staff, clinical area, Trust and so on. Please read the Confidentiality guidelines within this Assessment tab.
Presentation/formatting guidelines:
Your student roll number should be the title of the Word file. Do not insert your name anywhere in your assignment or on Turnitin.
Type your essay in font size 12 or larger and fully justify your formatted text. Number all pages.
Use line spacing 1.5 in the main body of your assignment and single line spacing in your final reference list at the end of your assignment.
Your ‘in-text’ citations and final reference list should be written in APA 6th (Harvard) style. Please use the University of Salford’s referencing handbook to avoid errors in referencing – see guide here
Provide a final word count (not including your final reference list) at the end of your assignment. The word limit is 3000 words.

4. To enable the student to examine the principles of asset-based approaches that support involvement of service users and carers in decision making
Word count
Your assignment should be 3000 words. This includes the introduction, main body of your assessment and conclusion. The word count does not include your final reference list. You must get as close to the word limit as possible. In accordance with the University’s Assessment and Feedback policy 2019/2020, if you do not adhere to the word count, assessors may determine that the task has not been completed in accordance with instructions and reflect this in the mark awarded.
Summative Feedback arrangements
Summative assessment and summative feedback – Marks and feedback shall be provided to students within 15 working days of the published assessment submission deadline on Tuesday 16 Feb 2021 at 16:00hrs. Your feedback and grade will be made available through the Voices & Choices Blackboard site. Use this guide to view grades and feedback.
Formative assessment and formative feedback – you will have a formative written assignment of 1000 words due for submission by Tuesday 17 November 20 by 16:00hrs. You can use your formative work (and your formative feedback) to work on your summative assessment. You will receive formative feedback on Tuesday 08 December 20 at 16:00.

Where there is a case of academic misconduct it will not be referred to FtP until the AMP case has been concluded, it will then be for the Dean’s nominee under FtP Procedure to make the formal decision on whether there are FtP aspects to consider relating to that outcome.
