504 Plan Assignment Directions

The main purpose of 504 Plans is to provide access to the general education curriculum for students with disabilities. Students with 504 plans typically have accommodations/modifications identified in the 504 plan to provide them with access to the general education curriculum. At times, schools may also use a 504 plan to provide related services. Below is a summary of the differences between a 504 Plan and an IEP.

Although special educators are not responsible for developing 504 plans you may find that your building principal or classroom teachers reach out to you for assistance since you have knowledge of accommodations and modifications to meet unique learner needs. In addition, those of you in the general education classroom or serving as a related service provider or reading specialist can also play a role in developing a 504 plan or advocating for a student you believe needs a 504 plan.

For this assignment, you will develop a 504 plan from a scenario student. A 504 plan form can be found in this module to fill in with the appropriate information. You may select the scenario that most interests you.

Scenario 1:

Brian is a junior with a 1.8 GPA. Last year he failed a semester of English 10 and a semester of Geometry. He is currently failing English 11, US History, and Algebra 2. His grades since 7th grade are mostly in the C to D range. He has met the standard on all of the state-required tests for graduation, and he was above the 50th percentile in all areas of the PSAT.

Brian has shared with his counselor that he often finds himself “zoning out” in class and is easily distracted all of the time. He often comes to class and is surprised when the teacher asks for everyone to turn in their homework because he wasn’t aware that there was a homework assignment. Brian’s parents have provided Guidance Team with a recent note from his doctor indicating that he has a diagnosis of ADHD.
