Description
This assessment relates to the following Learning Outcomes:
1. Examine mental health nursing from a strength’s perspective
2. Undertake a comprehensive mental state examination
3. Construct nursing interventions to assist in physical and psychosocial care of persons experiencing distress
Aim
This assessment gives you the opportunity to construct a Case Formulation, Mental State Examination and Risk Assessment.
1. Consider a client who you have engaged with in your clinical setting and provide a brief deidentified case history as well as a comprehensive Mental State Examination and Risk Assessment – PRESENT THESE IN A TABLE FORMAT AND PLACE IN YOUR APPENDICES.
2. Using this client, construct a Case Formulation utilising the 5P formulation model as a guiding framework and refer to your completed mental state examination and risk assessment
3. In constructing this assessment, it is important that you consider and integrate a mental health nursing lens and consider the types of nursing skills that may be required when undertaking assessments and formulating a plan.
Your assessment should include:
• A summary of your Case Formulation. The summary should explain how sections are inter-related.
• An analysis on how each section of the Case Formulation was derived. In this section you will need to draw on a completed Mental State Assessment and Risk Assessment, as presented in the appendix
• An explanation as to how mental health nursing skills and knowledge would be utilised in all aspects of your Case Formulation.
• Five “take home” lessons for the mental health nurse that have become evident while working through this unit of study and completing this assessment
Literature and references
In this assessment use literature of no more than 10 years old (from peer-reviewed sources) to demonstrate broad consideration of your alternate nursing interventions.
You may also use seminal scholarly literature where relevant.
Suitable references include peer-reviewed journal articles as well as textbooks and credible websites.
When sourcing information, consider the five elements of a quality reference: currency, authority, relevance, objectivity, and coverage. Grey literature sourced from the internet must be from reputable websites such as from government, university, or peak national bodies: for example, the Australian College of Nursing.
Requirements
• Use a conventional and legible size 12 font, such as Times New Roman or Arial, with 1.5 line spacing and 2.54cm page margins (standard pre-set margin in Microsoft Word).
• Include page numbers on each page in a footer.
• Use formal academic language
• Use the seventh edition American Psychological Association (APA) referencing style.
• Word count is calculated from the first word of the introduction to the last word of the conclusion. The word count INCLUDES in-text references and direct quotations.
• The word count does NOT include cover page, abstract, contents page, reference page and appendices.
