Jose is a 55-year-old Hispanic, male, migrant worker who speaks limited English. He presented to the emergency room with complaints of edema of the scrotum, urinary retention, and hematuria. He does not have any pertinent past medical history. He is married and has five young children. He states he has been having problems for a while but delayed seeing the doctor because of his work schedule and limited money. The doctor performs a digital rectal examination in the office and finds that Jose’s prostate is enlarged.

Based on the signs and symptoms Jose is exhibiting, it appears that Jose is suffering from an enlarged prostate, also referred to as benign prostatic hyperplasia (BPH). Benign prostatic hyperplasia affects 40% to 50% of men between the ages of 51 and 60 and over 80% of men older than 80 years old (Ignatavicius, Workman, Rebar & Heimgartner, 2021). BPH is a very common male health problem, but the exact cause is unclear.
BPH results from changes in the periurethral glandular tissue as a result of increased dihydrotestosterone levels resulting in inflammation. The inflamed prostate gland extends into the bladder resulting in bladder outlet obstruction, which can result in acute urinary retention (Cabrera & Schub, 2018). Jose is experiencing some of the signs and symptoms of BPH, including edema of the scrotum, urinary retention, and hematuria.
Other signs and symptoms of BPH include frequent or urgent need to urinate, nocturia, difficulty starting urination, weak urine stream or a stream that stops and starts, dribbling at the end of urination, inability to completely empty the bladder, and urinary tract infection.
To provide multidimensional care to Jose, I would first call an interpreter to ensure communication was accurate. Next, I would make sure Jose was comfortable by controlling his pain level and request a urinary catheterization be ordered if the urinary retention was severe enough. I would also ensure Jose has the support and education he needs to manage and cope with this diagnosis.
This may include referring Jose to a urologist or a social worker in his treatment and discharge routine. Encouraging Jose’s family to support and understand his diagnosis can also help ensure Jose is coping with it properly.
