– Domperidone 10mg Twice a day (For nausea/dizziness)
– Ondansetron 4mg Twice a day ( For nausea)
– Escitalopram 20mg Daily (For depression)
– Esomeprazole EC capsule 40mg daily (For GORD)
– Melatonin MR 2mg (For insomnia)
– Targin MR 5mg/2.5mg (For pain assoc. with RA and OA)
– Oxycodone Hydrochloride 10mg (For pain assoc. with RA and OA)
– Paracetamol Osteo 665mg x 2tabs Twice a day

– Rivaroxaban 20mg Daily (To prevent blood clots)

Perform a holistic, person-centered assessment addressing the promotion of health and wellness, as well as illness management. Ensure you analyse the assessment data and develop a nursing support plan using the guide in Theme Four ‘A
Nursing Support Plan’.
Your answer should include:
A comprehensive assessment of the client using appropriate assessment tools – this must include the client’s experience of chronic illness and its life impact, as well as identification of ‘what matters’ to your client
Assessment of the client’s level of health literacy and engagement in health care, health status using ICF domains, i.e., impairments, functioning, and participation
Identification of the client’s strengths, capacities, and knowledge and their goals
Strategies and interventions to achieve their goals – these will include nursing support and self-management.
