Integrated Care- Define integrated care. Explore integrated care, key evidence. National drivers- Who is leading integrated care.
Include: Discussion on NHS Long term plan. What are the identified benefits and to whom? What are the strengths and weaknesses?
Patients Pathway- Integrated care pathway. Include a diagram of your patient’s integrated care pathway
Elements of the Pathway- treatment options medical and nursing interventions – Evidence, National guidelines, including rationale. National guidelines- NICE guidelines and evidenced based practice
Nursing and bio-psychosocial frameworks and tools that underpin effective, inclusive, and person-centred assessment, planning and evaluation of care. Identify bio-psychosocial frameworks. What was your role in patient journey?
Discuss each aspect of your patient’s pathway- Role of MDT and integrated care services that your patient accessed, what was their role ?
Service improvement processes- Demonstrate your understanding of the research/ audit/ evaluation/ service improvement processes and their relevance to the delivery of high-quality care – relate to your patient’s integrated care pathway.
Evaluate the effectiveness of your patient’s integrated pathway- Evaluate the effectiveness of the integrated pathway for your chosen patient/ service user. Evaluate how integrated care benefitted/hindered the patient’s pathway /experience. (Strengths/weaknesses). Were there areas for improvement/recommendations.
Conclusion- Summarise the findings in your E-portfolio recommendations for practice. Do not introduce new ideas
