Assessment of the Older Adult
Objective
This assignment will give you the opportunity to review an assessment tool and to develop appropriate intervention and evaluation strategies related to your assessment.
Introduction
Standardized tools are an efficient and effective way to collect and organize data from the assessment of an older adult.
You will identify a relevant health issue pertaining to Older Adults. You will then choose a standardized tool that could be used to perform a detailed assessment of that issue (eg: Urinary Incontinence CHAMMP Tool). You will be expected to consider how the tool could be applied in practice, and provide analysis of your nursing assessment, interventions and evaluation.
Assignment Requirements
The body of your paper will be a maximum of 3-4 pages, excluding the title page, reference page &/or appendices.
Your paper must be supported with a minimum of 5 relevant and current RNAO BPG, evidenced based websites, scholarly articles or course text resources.
All references must be cited in the body of your paper and referenced at the end using APA formatting. Review the APA Guide for instructions on citations and referencing.
Your paper must be submitted to DC Connect at the identified due date and time in week 5. All documents must be written in Microsoft WORD format and will automatically be sent to Turnitin for originality verification upon submission to DC Connect.
Assignment Instructions

1. A brief introduction to the standardized tool that you would use to perform a focused assessment and approach related to the issue.
2. A description of how you would perform an assessment of the client using this tool.
3. Identify factors to consider when planning interventions for your selected issue.
4. Identify and describe nursing interventions to help the client in resolving or coping with the identified problem.
5. Describe how you would evaluate your selected interventions
6. Conclude your paper with why this assessment tool is valuable to nursing practice
