THE INTERVENTION I HAVE CHOSEN TO ADDRESS THE PROBLEM WILL BE INSTITUTING A NURSING CARE BUNDLE FOR WOUND PREVENTION FOR PATIENTS AT RISK FOR DEVELOPING HOSPITAL AQUIRED PRESSURE INJURIES OR SKIN BREAKDOWN. THE BUNDLE WILL INCLUDE NURSING INTERVENTIONS THAT WILL AUTOMATICALLY POPULATE BASED ON AN ALGORITHM CREATED FROM THE PATIENTS ADMISSION ASSESSMENT. IF THE PATIENT IS HIGH RISK FOR SKIN BREAKDOWN, THE NURSING BUNDLE WILL BE POLICY TO IMPLEMENT.
