The case study could be structured as outlined below:
Give a brief background of the case study including the underlying causes, pathophysiology and existing management of the long term condition(s). Outline the rationale of why you chose the patient in this particular case study.
Identify and discuss the role of the nurse in caring for a patient with single or multiple long term conditions, including medicines management.
Analyse the issues relating to encouraging self management and explore how nurses can empower your chosen patient and their families to be proactive in addressing these issues.
Case scenario
Patient X is a 60-year-old female diagnose 4 years ago with T2 Diabetes, She was admitted to the hospital due to fatigue, worsening blurred vision, infected foot wound and unintended weight loss. Patient not compliant with her medication and less exercise and not concerned with her diet. Patient medication for Diabetes ( metformin). She still working full time
