Assessment Brief
You will be completing a 2,500 word report. This assignment requires you to identify the process of planning care, looking at the stages from the process of referral to and exit from health and social care service, including the process of assessment.
The assignment requires you to identify the following:
Examine the stages of the care planning process, including the information needed for assessment, the input required from the Multi-Disciplinary Team (MDT), the importance of monitoring and review throughout the process.
Evaluate the Care Planning process with reference to relevant theory, demonstrating the importance of a Person-Centred approach throughout. Demonstrating how service users, their families and carers might be involved.
Provide an overview of relevant legislation, policy, guidelines in relation to current best practice when planning person centred care.
A flow chart of the process of care planning must be included , showing each stage and its importance, an example of a specific area of health and social care can be used.
