Service Plan: In an effort to justify to a parent or guardian the services the campus will
provide for their child; a service plan needs to be developed. Either from the Internet or
anonymous results from your campus find assessment results and develop a service plan
for an individual child.
Each plan should include the following:
Name of Instrument
Summarize Assessment Results
Identify Other Instruments Used to Validate or Challenge Results
Outline Conclusions (Identify Needs)
Make Recommendations for Service Plan
