Aim of the Module.
The aim of this module is to build on your prior learning to gain further knowledge and skills in working in partnership with people. You will continue to develop clinical decision making in relation to the management of long term, multiple and / or complex conditions. Key topics in this module will be integrated care and co-ordinating teams in and across different care environments.
Learning Outcomes
On successful completion of the module, you will be able to:
LO1. Reflect on the lived experience of the person living with long term care needs and analyse how a nurse may respond.
LO2. Debate strategies that may be used to coordinate the care of people with long-term care needs across different settings.
LO3. Explain how nurses work in partnership with people to empower, support and promote the health and well-being of individuals with a long-term condition.
LO4. Analyse strategies employed by nurses to manage risk and promote a safe and effective health care environment.
Written Assignment/Essay 3000 words Pass mark 50%Discuss how a nurse can effectively empower a person living with a long-term condition.
Summative assignment:
The 3000 word written assignment makes up 75% of the marks for this module.
The task
Discuss how a nurse can effectively empower a person living with a long-term condition.
Case study
Either choose a person who is living with a long-term condition to discuss from your own practice experience or use one of the case studies provided during week 1. Present the person in a text box at the start of your assignment (max. 250 words – not counted in the word limit).
Introduction: This should include a brief explanation of the long-term condition experienced by the person.
Main body of the essay:
Discuss how you would empower this person to live with their LTC including the following three themes:
– Self-Management
– Understanding the lived experience
– Working in partnership and models of care
Conclusion
Briefly draw together the findings presented in your essay.

Scenario for MH Self-Management
Bethany is 25yrs old she is currently in hospital due to a breakdown in relationship and an attempt to end her life. She has had numerous admissions to hospital over the past 3 years.
She is very angry about being in hospital again, she does not agree with her diagnosis of Emotionally Unstable Personality Disorder and does not want help for this. She feels that everyone is interfering with her life.
Bethany takes mediation while in hospital as it makes her feel calm but historically on discharge chaotically takes her medication using cannabis and alcohol instead.
Bethany dropped out of Uni she was hoping to be a teacher. She loves animals and did volunteer as a guide dog walker. She has a fragmented relationships with her parents, but is close to her older brother. She often has ‘friends of the moment’ and her latest best friend is Mel another patient on the ward.
Her mood is erratic and she only engages with certain staff members, one person she will talk to is the Peer Worker.
