Complete an episodic visit SOAP note for a patient whom you have seen this week. Use this SOAP note template. Do not use any identifiable information initials will suffice.
This should include all subjective, objective, relevant assessment findings, and plan. The plan will need to include relevant diagnostics, along with pharmacological and nonpharmacological interventions.
