Clinical Practice Flowchart for a Chronic Condition Instruction and Grading Rubric
You are a nurse practitioner in an outpatient primary care clinic. You have a full panel of patients of your own. To improve the quality of care, you are asked by your administrator to prepare a practice flowchart for one chronic condition that is most commonly encountered in your practice, such as such as Type 2 DM, hypertension, dyslipidemia, osteoarthritis, CHF, and COPD.
The administrator requests the following documents:
A chronic condition practice flow chart/ algorithm that is based on current practice guidelines
A supportive report explaining the flow chart
In order to prepare for this assignment, you would:
Select one chronic condition commonly encountered in clinical practice,
Thoughtfully review a current clinical guideline by a national or international organization thoughtfully
Create a practice flow chart that can be used in managing patients with similar diagnosis in your clinical practice.
Provide a supportive report to explain your practice flow chart.
Your supervisor will evaluate your assignment based on the following criteria (Grading Rubric) :
Chronic condition practice flow chart (15%)
The clinical setting in which your practice flowchart will be used (ie: rural health clinic or privately insured population) (5%)
What you would do at the initial appointment (ie: new patient) (30%)
Be sure to include history and physical, diagnoses and plans
What you would do for the 2nd appointment and 3rd appointment (follow up’s) (40%)
How do you determine if patient’s condition is adequately managed
What you would do if the condition is well managed or poorly managed
The basis for your decision making
Be sure to reference a current practice guideline, and comment on what makes this source scholarly? (5%)
Think of the author’s credibility, the evidence used and currency of the publication
Organization, clarity and creativity (5%)
