Whilst on your student placement within a supported home for people with a Learning Disability you are introduced to the service users by the staff. The Team Leader during hand over reports how Mark, one of the service users, continues to remain distressed and anxious over the past few days. One of the nurses’ states “oh he is only doing it for attention!”

Mark is thirty-two and has Downs Syndrome and a diagnosis of a mental health problem for which he is prescribed antipsychotic medication. He needs full staff support in the management of his medication. Over the coming week whilst you are here, Mark has become increasingly psychotic and his capacity to make decisions has changed. This has resulted in a Deprivation of Liberty (DoLs) being instated.

You are asked to help with the medication round and when you check Marks medication it is discovered that his antipsychotic medicines had not been ordered the previous month and did not show on the Medication Administration Record (MAR) chart.

Upon investigation it is found that the due to a procedural error when ordering medication Marks’ antipsychotic medication disappeared off his prescription and therefore has not been administered. This has resulted in the deterioration in Marks mental health.
This prolonged deterioration in his mental health has resulted in prolonged psychological harm towards Mark.
