Assignment : Guideline
Description: There are multiple care delivery models, such as team nursing, primary nursing, functional nursing, case management and so forth.
Step 1: Develop a care model to apply within one of the following departments: medical, surgical, labour & delivery, emergency, mental health, or palliative/hospice.
You may also choose another department, but get it cleared with your instructor to ensure it will meet assignment expectations.
After selecting a department, identify which care model is currently being used on the unit of your choice. develop a different care model to apply to the clinical setting you chose.

Step 2: Within a 5-page max paper (not including title page and references), discuss the following: Use the following questions as subtitle to write a 5 page max paper.
Explanation of the problem you are addressing (unique to the unit you have chosen)
Problems addressing include
Longer wait time in emergency
Overworking staffs
Staff shortages
Bed shortage
Small working area
Decrease patient satisfaction
Think about the care of that unit. What are the challenges? What is the current model? What are the shortfalls of the model? What issues are present? What goals do you have to deal with those issues?
What care model have you chosen? (Care model chosen – Synergy Care Model) Is it a hybrid, modified or new model?
Staff mix you will implement with your care model (UC, HCA, LPN, RN etc.)
Is there utilization of health professional’s full scope of practice?
Workload/patient allocation
Expected outcomes for patients (Health Care Quality) and staff
What is the anticipated effect on the budget?
Is there utilization of electronic health records, robotics, telehealth, or artificial intelligence?
How do you plan to increase staff buy-in to your newly rolled out care delivery model?
