Comprehensive Nursing Care Plan
The purpose of this care plan and SBAR summary is to ensure that you demonstrate a thorough understanding of the patient’s primary and secondary diagnoses that includes their etiology and pathophysiology. This assignment is also intended to see your implementation of a primary and secondary assessment into your treatment plan, including necessary diagnostic tests,
, therapeutic interventions and to ensure that you can identify key nursing
assessments and interventions. The care plan should include best-available evidence.
Instructions
1. Thoroughly review the three scenarios presented below.
2. Choose one of these scenarios to create a plan of care make sure to indicate, on your assignment template which scenario you are using.
3. Develop a comprehensive plan of care using the guideline provided below.
4. Create an informative summary, using the SBAR format, situation, background,
assessment and recommendations) that would be used to call the physician about the
patient scenario you chose. This is limited to one page.
Care Plan Expectations
• This care plan should be substantive, comprehension and based on evidence based
practices. Resources should include the use of peer-reviewed articles, clinical practice
guidelines as well as other scholarly resources such as textbooks, drug and pharmacology guides, and laboratory and diagnostics manuals.
• The care plan should not include direct quotes, please paraphrase information. Copying information directly from textbook and or academic sources is plagiarism.
• The care plans are to be formatted using the template provided and must referenced appropriately using APA 7th edition guidelines (in-text citations, reference page)
• The care plan must be written in full sentences. Check your spelling and grammar before submitting.
• Care plans are limited to five (5) pages. Any work that exceeds the page limit will not be read and not graded.
• SBAR summary report is limited to one (1) page. Any work that exceeds the page limit will not be read and not graded
• References should not be more that 2 pages and must be formatted using APA 7th edition guidelines. References must be scholarly academic references (peer-reviewed articles), and Canadian clinical practice guidelines.
