Project: Application of Interprofessional Theory to Vulnerable Populations
1. Analyze current programs/interventions in place to improve this health disparity of this vulnerable population and suggest possible solutions to address the health care disparities, utilizing the two theories.
a. What is currently being done for this population and this health issue? Is it
adequate? What needs are being addressed? What needs are currently still unmet?
b. Provide a brief description of how the two theories could help better address
these disparities by driving the development of interventions to resolve them.
Vulnerable population is the Homeless and health disparities can be mental health, housing etc.
2 theories/models of care that can be used are:
1. Health Care Home Program (HCH)
2. Mobile Health Clinic to make it easier to access them
