During my recent clinical experience as an APRN student in a primary care setting, I had the opportunity to assess and care for a 45-year-old male patient who presented with complaints of chronic lower back pain and difficulty with mobility. This clinical encounter presented both challenges and valuable learning experiences.


One of the primary challenges I faced during this clinical experience was the need to establish a trusting and open rapport with the patient, considering his chronic pain condition, which can be complex and deeply personal. Additionally, addressing chronic pain often requires a multifaceted approach that includes both medical and holistic interventions, making it necessary to consider the patient’s physical, psychological, and social aspects.


A significant success of this clinical experience was the opportunity to conduct a comprehensive assessment of the patient, integrating knowledge and skills gained through my APRN training. Effective communication allowed me to understand the patient’s pain history, lifestyle factors, and goals for managing his condition.

Patient Assessment

Signs and Symptoms (S&S)

  1. Chronic Lower Back Pain: The patient reported persistent lower back pain, which had been ongoing for several years.
  2. Limited Mobility: He expressed difficulty with daily activities such as walking, bending, and sitting for extended periods.
  3. Radiating Pain: The pain occasionally radiated down his right leg, suggesting possible nerve involvement.
  4. Psychological Impact: The patient described feelings of frustration, anxiety, and depression due to his chronic pain condition.
  5. Previous Interventions: He had tried over-the-counter pain relievers, physical therapy, and chiropractic care with limited relief.


Based on the signs and symptoms presented by the patient, I considered several possible differential diagnoses:

1. Degenerative Disc Disease

  • Rationale: Chronic lower back pain with radiating symptoms is often associated with degenerative changes in the intervertebral discs. A thorough physical examination and imaging studies may help confirm this diagnosis (Anderson & Harris, 2022).

2. Lumbar Radiculopathy (Sciatica)

3. Psychological Factors and Chronic Pain Syndrome

Health Promotion Intervention
