Research your population. Using data from the CDC, LCDH, ODH, NIH, etc. assess common health problems for your population. Combine this information with your assessment data to identify the needs of your aggregate. Use one of the following Epidemiology models:
Epidemiological Triangle or Theory of Multiple Causation (Web of Causation) and apply them to your population and their health risk. This will include the distribution (Who, When and Where) and determinants (WHY – what are some factors that may have impacted or caused the problem?)
Discuss how the model best addresses the issue you have identified as the health problem or risk and define its relation to the aggregate you have chosen.
Create a diagram of the model to illustrate how the model applies to the topic and aggregate
